Wound Care

For patients with hard to heal wounds, our comprehensive wound care program includes a broad range of leading edge therapies and treatments, including:

  • Surgical wound debridement
  • Wound VAC
  • Burn care
  • Chemical wound debridement
  • Tissue ultrasound
  • Electrical stimulation

Our full-time staff includes a physician wound care specialist and trained wound care nurses who play an active role in assessing, diagnosing, and overseeing all wound care procedures and protocols. To prevent recurrence, our unique whole person approach to care also stresses wound prevention and self-management.

The program is designed to treat a wide range of problems, including:

  • Diabetic ulcers
  • Pressure ulcers
  • Arterial and venous ulcers
  • Burns
  • Infections
  • Surgical wounds
  • Amputations

Also treated are complex wounds resulting from conditions that
compromise circulation, such as: diabetes, traumatic injury,
surgical breakdown, congestive heart failure, vascular disease,
incontinence, obesity, renal failure, lymphedema and others.

Tracheotomy Care

We offer tracheotomy care to residents and patients with severe lung disorders, neurological problems or infections, who are dependent on a surgically inserted tube (trach) for breathing. In addition to continuous nursing, we perform daily trach maintenance and comprehensive respiratory services in order to minimize complications, maintain stability, and return the individual to his or her highest level of functionality or independence.

Stroke Care

The goal of our specialized stroke program is to restore residents to their highest level of functionality, help compensate for decreased capabilities, regain greater mastery over tasks of daily living, and achieve an improved quality of life.

Our highly personalized, integrated programs incorporate, physical, cognitive and psychological components, including:

  • Physical, occupational and speech therapy
  • Gait, balance and mobility training
  • Psychological support
  • Dysphagia therapy

Restorative Nursing

For those patients who are no longer in an acute or short-term rehabilitation program, restorative nursing can help close the gap between rehab and home. Directed and implemented by a registered nurse and specialized aides, restorative nursing focuses on the therapeutic use of simulated life activities to help restore daily living skills.

Respiratory Care

For patients coping with the often devastating effects of lung disease and other chronic respiratory problems, our comprehensive respiratory therapy program provides hope and help.

In addition to individualized physical and occupational therapies, treatments may include:

  • Oxygen therapy
  • Dietary counseling
  • Education in disease management
  • Self Care Instructions
  • Breathing control techniques

Rehabilitation

Unlike many area facilities, we offer physical, occupational and speech therapy up to seven days a week, for up to three hours a day, as ordered by the resident’s physician. As part of an integrated treatment program, these advanced therapeutic services address a wide range of orthopedic, neurological and other disabilities, diseases and conditions, including:

  • Joint replacement
  • Neuro-muscular diseases
  • Cardiac conditions
  • Traumatic brain injury
  • Spinal cord injury
  • Stroke
  • Dysphagia
  • Fractures
  • Post-surgical complications
  • Diabetes complications
  • Amputation

Nutritional Management

For residents and patients with nutritional disorders, as well as those with weight management issues, we offer a comprehensive nutritional management program.

Individualized plans are developed for both risk reduction and/or management of the medical conditions impacted by an individual’s nutritional status, including:

  • Diabetes
  • Cancer
  • Cardiovascular disease
  • Hypertensive disorders
  • Lipid disorders, and
  • Gastrointestinal dysfunction

When focused on the management of obesity, services include:

  • Nutritionist-designed meal planning
  • Exercise
  • Nutritional education
  • Counseling and behavioral support needed to accomplish long-term weight management goals.

Neuromuscular Care

For persons affected by neuro-muscular disorders, including Multiple Sclerosis (MS), Guillain-Barre Syndrome and Parkinson’s Disease, our facility provides comprehensive care designed to restore maximum functional performance and overall well-being.

Individualized care plans address each patient’s physical, cognitive, emotional and vocational needs and deliver a range of specialized treatments and services, including:

  • Strength and range of motion exercises
  • Mobility training
  • Re-education in skills of daily living
  • Communication skills re-training
  • Swallowing management
  • Gait and balance training
  • Psychological counseling and family support
