Sub-Acute Care

Bridging the gap between hospital and home, sub-acute care continues care for the patient who is over the acute phase of illness but still needs medical care and rehabilitation. This section of our facility offers warm, personal care from professional staff members for patients who require daily treatments in accordance with special needs for rehabilitation, wound care, I-V therapy, pain management, or post-operative care and recovery, including self-care training for medications, dressings, etc.

Semi-private rooms are equipped with electric beds, telephones and televisions at each bed. Each room has toilet facilities with mirror and sink for the convenience of all patients, as well as handicap accessible showers. A call system at each bedside connects to an intercom at the centrally located nursing station, allowing the resident to talk directly to the nurse.

A Typical Day

Your day at Okeechobee Health Care Facility is designed to increase your independence in daily living activities. Recreational therapy is provided individually and in groups throughout the day and several evenings. Check the daily & weekly activities calendar on Channel 2 of your television posted in your room and on the unit. You may have rest periods during the day where you can read, watch TV, socialize. Your family, children, grandchildren  may visit you at any time of day or night; friends may visit daily from 8:00 a.m. until 8:00 p.m.

You will awake between 7:00 & 8:00 a.m. You will be provided with instruction, training and assistance as needed for bathing, hygiene and dressing. You will dress in your own clothes. After breakfast you will participate in rehabilitation therapies as prescribed by your physician. You may be scheduled in the morning and/or afternoon.

Books, newspapers, magazines and board games are available throughout the facility. Wireless internet connections are also available throughout the facility for your laptops or mobile devices, and residents have access to computers in the Sun room and in the Main Lobby. The Admissions personnel can provide you with the necessary passwords for access.



A Team Approach

The team will work with you to set goals and design an individualized plan of care to meet your rehabilitative and medical needs. Team members envision the care of the resident and their family as one process, and will meet weekly to discuss your overall progress and future goals. The team will work with you and your family to coordinate care and discharge plans.At Okeechobee Health Care Facility, an interdisciplinary team will assist you to become more independent in daily living activities and achieve your health care goals.  This team will include doctors, nurses, therapists, dietitians, social workers, and other professionals as needed.

All professional staff are graduates of schools, colleges and universities approved by nationally recognized accreditation bodies. Professional staff meet current legal requirements of licensure, registration, certification and competence.
